Patient intake forms

A QR code in the waiting room that opens your intake form on the patient's own phone. They fill it out before they see the provider. Works with any form tool: Google Forms, JotForm, or your EHR's patient portal.

Appointment reminders

Include a QR code on printed appointment cards that links to your patient portal or a reschedule form. Reduces no-shows when patients can confirm or cancel without calling the front desk.

Prescription and medication info

Link a QR code on medication instruction sheets to a plain-language explainer page, a pharmacy FAQ, or a video walkthrough for complex administration. No PHI involved, just educational content.

Patient satisfaction surveys

A QR code on the discharge paperwork or checkout receipt linking to a short satisfaction survey captures feedback while the visit is still fresh. Better response rate than a follow-up email three days later.

Contactless check-in

Patients scan a QR code at the door to notify the front desk they have arrived. No waiting at the window, no shared clipboards. Works well for practices with outdoor or lobby waiting.

Adding QR codes to your patient workflow

Test your destination page on a phone first. Most patients are scanning on iPhone or Android, often older devices. If the page is hard to read on a 5-inch screen, fix that before printing the QR.

Choose the QR type: URL for intake forms and surveys, Phone for after-hours triage, SMS for appointment reminders.

Make sure the destination requires no app download. Patients will not install something to fill a form.

Print at least 2 inches (5cm) square. Place at eye level near reception and exam rooms. Add a small printed instruction line under it for less tech-savvy patients.

Are QR codes HIPAA compliant?+

A QR code is just a way to open a URL. It is not inherently HIPAA compliant or non-compliant. What matters is where the code leads. If the destination page requires patients to log in to a secure, encrypted portal, and that portal is hosted by a HIPAA-covered entity with a BAA in place, then the overall system can be HIPAA compliant. The QR code itself carries no PHI.

What should I never put directly in a QR code in a medical setting?+

Do not embed patient names, dates of birth, appointment details, diagnosis codes, or any identifiable health information directly in the QR code content. Those things should live behind an authenticated login on a secure platform. The QR code should only ever contain a generic URL or public information.

Many of our patients are elderly. Will they struggle to scan QR codes?+

Some will, some will not. A 2023 Pew survey found that 61 percent of adults over 65 own a smartphone, and QR code literacy improved significantly post-pandemic when restaurants adopted them widely. That said, always provide an alternative: a phone number, a paper form, or staff assistance. QR codes should reduce friction for patients who want them, not create a barrier for those who do not.

Can we offer QR codes in multiple languages for diverse patient populations?+

Yes. Create separate QR codes for each language version of a form or information sheet. Label each code clearly (in the target language) so patients can self-select. Some form platforms like Typeform support language detection automatically, so one QR code can serve multiple languages if the form is built to handle it.

Our forms and portal links change sometimes. Do we need to reprint everything?+

If you use a static QR code and the URL changes, yes, you need to reprint. The easiest solution is to use a stable URL on your own website that you update in place (e.g., yourclinic.com/intake-form). The QR code stays permanent; you just update the page it points to. This avoids reprinting posters and handouts every time your form platform changes.

Is a QR code a secure way to share a landing page link?+

A QR code is as secure as the URL it contains. If the URL uses HTTPS and the page requires authentication, scanning the code is no less secure than clicking a printed link. The code itself is publicly readable by any camera, so do not put anything in the QR code content that you would not also print in plain text on the same sign.
